address___________________________________________________________________________________________________________________________________________________________________________________phone_________________________________________email_________________________________________hereby make application for a course of instruction entitled: Linnaea Farm Ecological Gardening Programme (LGP), commencing at the beginning of March each year and concluding at the end of October, and consisting of thirty four (34) weeks of instruction. This course of instruction is to be taught as set out in the course outline and is duly registered under the provisions of the Private Career Training Institutions Act.
I understand that there is a non-refundable registration fee of $100 due in mid Dec. of the preceding year. Tuition is specified in Linnaea advertising and I agree to make payments of roughly 45% of the total, due on the first day of the course, and two installments of about 22,5% of the total each, due at the beginning of May and July. I understand that there is an additional charge for the Linnaea text, "Selected Readings" of $75.00. Additional materials required for the programme, which must be acquired from other sources, are outlined on the attached list and may cost from $85 to $500.
I have read and understand LGP's refund policy enumerated below:
REFUND POLICY
LINNAEA FARM ECOLOGICAL GARDENING PROGRAMThis policy applies to both Canadian and International students.
REFUNDS IN THE CASE OF WITHDRAWAL OR DISMISSAL1. written notice must be provided by
.....1a. a student to LGP when the student withdraws
.....1b. LGP to the student if the student is dismissed.
2. Refunds before the course begins. If written notice of withdrawal is received by LGP any time after the contract is made, and before the start of the programme of study, LGP may retain the $100 registration fee.
3. Refunds after the programme of study starts.
.....3a. If written notice of withdrawal is received by LGP or a student is dismissed during the months of March or April, LGP may retain 30% of the total tuition fees due under the contract.
.....3b. If written notice of withdrawal is received by LGP, or a student is dismissed during the months of May or June, LGP may retain 50% of the total tuition fees due under the contract.
.....3c. If a student withdraws or is dismissed after June 30, no refund will be given.
4. Refunds owing to students must be paid within 30 days of the institution receiving written notice of withdrawal or within 30 days of LGP's written notice of dismissal.
REFUNDS IN THE CASE OF INSTITUTION CLOSUREShould LGP close, voluntarily or otherwise, before the full programme of study is delivered, the Executive Director of the Private Post-Secondary Education Commission may order a refund of all or part of paid student fees.
REFUNDS IN THE CASE OF UNDUE CHANGES TO THE PROGRAMME OF STUDYShould LGP close, voluntarily or otherwise, before the full programme of study is delivered, the student may be eligible for a partial or full refund through the Student Training Completion Fund.
When an international student* enrolled with LGP on the basis of a Study Permit withdraws or is dismissed, LGP is required to notify Citizenship and Immigration Canada within fourteen (14) calendar days that the student is no longer enrolled.
Where a student is deemed not to have met the LGP minimum requirements for admission, LGP must refund all fees paid under the contract.
I agree to be in regular attendance, and to carry out the instructions of the management and instructors during programme time.
I understand that LGP will be operated in conformity with the Private Career Training Institutions Act and PCTIA bylaws. I further state that I have read and received a copy of the
Policies & Procedures of the Linnaea Farm Ecological Gardening Programme.
In accordance with Part 4(10)(1)(a) of the
Personal Information Protection Act, we hereby notify you that your name and personal identification information, the name of your program of study, and the amount of the tuition paid will be forwarded to the Private Career Training Institutions Agency for the purpose of administering the Student Training Completion Fund. This information is collected by the PCTIA under section 26 of the Freedom of Information and Protection of Privacy Act.
